Some background: I met this memory exception in SSMS 2012, since it was released with Red Gate SQLPrompt (this exception never occurs in my SSMS 2008R2 on the same laptop).
Initially, I came across these exceptions (SSMS2012 and SQLPrompt), which made me close and reopen SSMS (along with everything I worked on). A few months ago, I came across a niche of support that pointed to a possible add-on as the reason, so I removed my only add-on (SQL Prompt, but did not remove the rest of the development package) and went many months without exception, with the exception of a System.OutOfMemoryException exception.
As soon as SP1 was released in 2012, I applied it and installed SQL Prompt (latest version) again to find out if the problem was affected, and within 7 hours from the moment of development I was again met with the notorious exception System.OutOfMemoryException.
From all this, I opened tickets with Red Gate and sent debug logs where they saw the exception, but since the memory exception did not explicitly display SQLPrompt, they would not escalate the problem to the development team. Prior to this specific exception, SQL Prompt, however, throws many exceptions (some of which are listed below) in the SSMS 2012 IDE (Visual Studio 2010). I believe that memory exclusion is a symptom of the problem with how SQL Prompt manages cached data and feeds on available memory for SSMS, which ultimately throws an exception as a result.
