Using bits of code code / other peoples on the dissertation

I am writing a dissertation for the last year at the university. I want to add a lot of source code to a DVD for an application. 95% of the code is written by me, but some code is copied and pasted from samples and taken from the forums.

Do I need to mark every bit of code that is not mine or anything else? What would I write if I do not know who his code is more?

You must indicate the source, if the source indicated that you are doing this. However, if you take samples and adapt them to your environment, you usually do not need to refer to it (as long as it has changed significantly).
